If you have children and they are getting the H1N1 vaccine at school, download this form and have it completed at home so your child may take it to school with them.

| Food handling/safety classes will be held the following dates in 2010. All classes start promptly at 3:00 pm and are held at the Cooper County Public Health Center located at 17040 Klinton Drive, Boonville. Supervisor class will last about 2 hours, Basic about 1 hour. Please call 660-882-2626 to make reservations or with any questions. |

Equipment For Donation
The Cooper County Board of Trustees has developed a policy that retired office equipment may be distributed to non-profit agencies for free. If after 30 days any remaining equipment may be disposed of through proper sanitation methods.
